In order to accommodate all of our bowlers, we will assign four people to each lane. Please answer the following questions:

Each registered bowler is asked to raise at least $75.00 in order to participate. You can download a pledge sheet by clicking here. Checks should be made payable to: Big Sisters of Rhode Island. All checks/cash should be collected before the bowl-a-thon and given to Big Sisters the day of the event. All donors of $5.00 or more will be sent a thank you letter by Big Sisters of Rhode Island, so please be sure to include name, address, and amount given for each donor on the pledge sheet, which you should turn in the day of the event along with the money collected. All donations are tax deductible. In consideration of my participating in Big Sisters of RI bowl-a-thon, I hereby waive and release Big Sisters of RI, its officers, directors, employees, agents and assigns and AMF Cranston Lanes, Old Mountain Lanes, and Walnut Hill Bowl from all claims for damages which I may have against any or all of them, for personal injury or property damage that may result in any way to me or any minor child with me participating in the bowl-a-thon. This waiver and release shall bind my heirs, executors, administrators, and assigns as well as myself. Please make a selection. Agree
